We ask that you review and complete all intake, ,medical history, and consent forms online prior  to your visit to our office.

If you are coming in for a consultation, we prefer that you don not take pain medication beforehand, so we can better assess your symptoms. 

If you've already been diagnosed and scheduled for treatment at our office we recommend taking one hour before your appointment or as directed:

• Ibuprofen (Advil, Motrin) 
• Acetaminophen (Tylenol)
• Naproxen Sodium (Aleve)

Wear loose-fitting, comfortable clothing.  Be prepared to remove all metal jewelry and piercings located  above chin-level.

All patients under the age of 18 must be accompanied by and have all intake forms signed by a parent or legal guardian.

Antibiotic pre-medication - Please inform us if you have been advised by your physician to take antibiotics prior to dental procedures. Common conditions include infective endocarditis, joint replacement surgery, and compromised immune system.

Anxiety - We find the vast majority of patients do not require pharmacologic strategies to manage anxiety.  However, in select cases, options include

• Oral Conscious sedation, which is taken in the form of a pill.  You are required to have someone drive you to and from your appointment.

• Nitrous Oxide - aka "laughing gas".  No escort is necessary

• Intravenous Sedation is much deeper sedation.  We would arrange for a dental anesthesiologist to administer medications at the time of your treatment  - you are required to have someone drive you to and from your appointment.

Imaging (2D and 3D), clinical testing and examination with an operating microscope along with pertinent medical and dental history will be taken together to reaffirm or formulate a diagnosis and plan, which may, but not always, involve endodontic treatment.

In many cases, treatment can be performed at the initial visit if indicated.
